Orthopaedic and Spine Center is excited to partner with MedRecs Management, based in Nashville, TN, for medical record requests and disability/FMLA starting on January 15, 2024.
Our partnership with MedRecs Management will allow for secure and fast turnaround times for our patients requesting medical records and disability/FMLA form completion.
Here’s how patients will access the medical records and disability/FMLA process once we are live:
•On the Orthopaedic & Spine Center website (www.osc-ortho.com) click “Patient Center.” There will be a “Request Medical Records” and “FMLA/Disability Forms” link. Follow the simple instructions to submit your request.
•Orthopaedic & Spine Center will also have flyers to distribute to patients who want to know how to request a medical record and/or disability/FMLA request. The flyer will include:
o Instructions on where to go on the Orthopaedic & Spine Center website to access the form completion page
o A QR code that sends the patient directly to the form requesting page
o A phone number specific to Orthopaedic & Spine Center [(757) 244-9502] if patients have questions about completing the request. This number routes to a domestic, live MedRecs Management team member. If a MedRecs Management team member is assisting another patient, you must leave your name and callback number to receive a same day callback.
